In order to enhance the “structural competency” of medicine—the capability of clinicians to address social and institutional determinants of their patients’ health—physicians need a theoretical lens to see how social conditions influence health and how they might address them. We consider one such theoretical lens, fundamental cause theory, and propose how it might contribute to a more structurally competent medical profession. We first describe fundamental cause theory and how it makes the social causes of disease and health visible. Deep brain stimulation of the subthalamic nucleus or the globus pallidus is an established treatment for Parkinson’s disease that yields a marked and lasting improvement of motor symptoms. Yet, DBS benefit on gait disturbances in PD is still debated and can be a source of dissatisfaction and poor quality of life. Gait disturbances in PD encompass a variety of clinical manifestations and rely on different pathophysiological bases. While gait disturbances arising years after DBS surgery can be related to disease progression, (...) early impairment of gait may be secondary to treatable causes and benefits from DBS reprogramming. In this review, we tackle the issue of gait disturbances in PD patients with DBS by discussing their neurophysiological basis, providing a detailed clinical characterization, and proposing a pragmatic programming approach to support their management. We investigate the underexplored question of when speakers make use of the omission phenomenon verb phrase ellipsis (VPE) in English given that the full form is also available to them. We base the interpretation of our results on the well-established information-theoretic Uniform Information Density (UID) hypothesis: Speakers tend to distribute processing effort uniformly across utterances and avoid regions of low information by omitting redundant material through, e.g., VPE. We investigate the length of the omittable VP and its predictability in context (...) as sources of redundancy which lead to larger or deeper regions of low information and an increased pressure to use ellipsis. We use both naturalness rating and self-paced reading studies in order to link naturalness patterns to potential processing difficulties. For the length effects our rating and reading results support a UID account. Surprisingly, we do not find an effect of the context on the naturalness and the processing of VPE. We suggest that our manipulation might have been too weak or not effective to evidence such an effect. (shrink)

The novel literally opens with the words of another: “Das Vergangene ist nicht tot; es ist nicht einmal vergangen,” a slightly altered translation of a line from Faulkner’s Requiem for a Nun: “The past is never dead. It’s not (...) even past.” 1 Lacking a proper opening and haunted by the specter of an unmastered and perhaps unmasterable past—of a “model childhood” spent in the East Prussian town of L. during the Third Reich, the text begins by questioning the very possibility of a clean break and a fresh start. This insistent question, raised in a variety of ways throughout the novel, is posed at the outset as a kind of textual stutter, as a perseverating question rehearsed from the beginning—and in lieu of simply beginning—as a series of false starts, as repeated attempts to start over, as a desperate struggle to restart a narrative paralyzed by the anticipation of difficulties still to come. “You lay aside stacks of tentatively filled pages, insert a fresh sheet in the typewriter, and start once again with Chapter 1. Before 1933 commentaries on laws were exclusively juristic texts, written and read only by legal professionals. Beginning in 1934, scholars from different disciplines, especially medical scientists, began writing juristic commentaries. The essay examines the reasons for this development and explores how it changed the genre, using the example of the most important commentary on theBlutschutz-andEhegesundheitsgesetz, which resulted from the collaboration of two medical professionals and a legal professional. The article argues that the recruitment of non-juristic authors and the corresponding methodological (...) changes were caused by a debate on a possible “reform” of juristic commentaries which took place in law journals. From the perspective of the National Socialist regime, the genre in its traditional form was not compatible with the Third Reich’s philosophy of law.In this new genre, commentaries not only referred to legal problems, but also dealt with issues from other disciplines, such as medical questions. The essay modifies Mitchell G. Ash’s theoretical approach regarding the “entanglement” of jurisprudence and other disciplines. Law and other sciences became, according to Ash, “resources for one another”: Legal scholars could claim that legal decisions were based on scientific facts and therefore “unquestionable.” The affected disciplines also gained greater importance as a result of this process in which their findings becoming legally relevant. This transformation also enabled them to acquire resources for new research projects.Moreover, the essay examines the connection between this development and the change of audience within German courts. Not only did the authors of the commentaries change, but also the recipients: Many laws passed after 1933 introduced new courts using lay judges, professionals with a non-law background. The everyday realities of educational practices of the Third Reich are reconstructed in the memoires of involved observers of these processes. The most of them can be used as a factual supplement to theoretical reflections on totalitarian transformations in education as their subjective perceiving. Despite of different origin and life attitudes all the authors of translated fragments are concentrated on those features of totalitarian educational innovations which show their completely incompatibility with the humanistic tradition in education. The everyday life of (...) universities’ and school’s communities in the Third Reich was determined by the national-socialist ideology. The recalling on Heidegger’s activities as the rector of the University in Freiburg (H. Gottschalk, H. Jonas, K. Löwith, G. Cesar) expose the ambiguity of his way of thinking and acting, what was also noticeable in his habitus. His nationalism was not combined strong with the anti-Semitism in the university’s management. The race theory as a part of national-socialismideology wasn’t definitive for the everyday life in those educational institutions, where the educational traditions were connected with the humanistic values existing in families (L. Schmidt, G. Cesar). Some attempts to stimulation of the pro-social behavior of pupil and students (helping and solidarity) were not effective in the Third Reich because of their directive nature (G. Cesar). The comparison of the national-socialism model of the school and the Lichtwark School taking by L. Schmidt demonstrates the advantages of non-indoctrinated educational institutions withthe pedagogical and socialfreedom used for the all-side development of pupil personality. The experience of the membership in BMD (League of German girls), connected with the force working is critically analyzed by G. Cesar and L. Schmidt. The social status of women and their educational influence in the family of the Third Reich design is reconstructed by B. Vinken. She shows that the fascist ideology provides only the subordinated role of women in all spheres of the social life including the educational practices. (shrink)

Der folgende Text geht am Beispiel des maßgeblichen, von zwei Ärzten und einem Juristen verfassten Rechtskommentars zum Blutschutz- und Ehegesundheitsgesetz von 1935 der Frage nach, wie sich dadurch die Gesetzeserläuterung veränderte. Hintergrund ist eine ab 1934 in den juristischen Fachzeitschriften geführte Debatte um eine Reform des Genres Gesetzeskommentar, das in seiner herkömmlichen Anlage als inkompatibel mit dem nationalsozialistischen Rechtsdenken galt.Der (...) Aufsatz versucht diesen Prozess in Ausweitung des Theorieansatzes Mitchell G. Ashs als „Verflechtung“ des Rechts mit anderen Wissenschaften zu begreifen, die zu beiderseitigem „Vorteil“ war: Auf der einen Seite konnte sich die Jurisprudenz auf vorgeblich „wissenschaftlich-objektive“ Befunde berufen, die, anders als juristische Abwägungen – so die Suggestion –, als „nicht hinterfragbar“ angesehen wurden. Andererseits verlieh die unmittelbare Rechtsrelevanz bestimmter Befunde aus den betreffenden Wissenschaften diesen Disziplinen insgesamt einen höheren gesellschaftlichen Stellenwert, der wiederum die Erschließung neuer Forschungsfelder ermöglichte.Geprüft wird außerdem ein möglicher Zusammenhang mit einer partiellen Veränderung der Gerichtslandschaft: Mit der verstärkten Gesetzeskommentierung durch Nicht-Juristen erweiterte sich schließlich auch der Adressatenkreis der betreffenden Schriften. One aspect of auditory scenes that has received very little attention is the level of diffuseness of sound sources. This aspect has increasing importance due to growing use of amplification systems. When an auditory stimulus is amplified and presented over multiple, spatially-separated loudspeakers, the signal’s timbre is altered due to comb filtering. In a previous study we examined how increasing the diffuseness of the sound sources might affect listeners’ ability to recognize speech presented in different types of background noise. Listeners (...) performed similarly when both the target and the masker were presented via a similar number of loudspeakers. However, performance improved when the target was presented using a single speaker and the masker from three spatially separate speakers but worsened when the target was diffuse, and the masker was compact. In the current study, we extended our research to examine whether the effects of timbre changes with age and linguistic experience. Twenty-four older adults whose first language was English and 24 younger adults whose second language was English were asked to repeat non-sense sentences masked by either Noise, Babble, or Speech and their results were compared with those of the Young-EFLs previously tested. Participants were divided into two experimental groups: A Compact-Target group where the target sentences were presented over a single loudspeaker, while the masker was either presented over three loudspeakers or over a single loudspeaker; A Diffuse-Target group, where the target sentences were diffuse while the masker was either compact or diffuse. The results indicate that the Target Timbre has a negligible effect on thresholds when the timbre of the target matches the timbre of the masker in all three groups. When there is a timbre contrast between target and masker, thresholds are significantly lower when the target is compact than when it is diffuse for all three listening groups in a Noise background. However, while this difference is maintained for the Young and Old-EFLs when the masker is Babble or Speech, speech reception thresholds in the Young-ESL group tend to be equivalent for all four combinations of target and masker timbre. This article contends that Georg Simmel attempted a rehabilitation of the Jewish stereotype in a singular way: via his theory of modernity and the quintessential place held therein by money. The first part of the article, based almost entirely on Simmel's The Philosophy of Money, seeks to demonstrate that Simmel intended to overturn the negative Aristotelian and Marxist assessments of money and of those who deal with it. The second part of the article is based on Simmel's unique theory of (...) type. This part seeks to establish that what Simmel in fact rehabilitates is the Jewish type, rather than any actual historical entity, and that this type is in fact the stereotype of the Jew. Simmel's attempt is made in terms of existential authenticity and in direct relation to the project of modernity. (shrink)
